ScoreHealth software guides the encounter and ensures that an effective and comprehensive assessment can be conducted in as little as five minutes. Using an on-line/off-line design similar to leading email software, care counselors and health educators can take ScoreHealth software to doctors' offices, worksites, schools and community clinics. This portability enables the counselor to conduct ScoreHealth encounters with people who are often elderly, under-educated, and living and working in underserved areas. This greatly expands these populations' access to healthcare and educates them in managing chronic disease and improving their overall health.
ScoreHealth software reports include individual and population-based summaries, in cross-section and over-time. Reports are available for senior management, program directors, counselors and clients. Users can produce pre-defined summary and detail reports as well as reports that they customize
to their specific needs. Also users can set client and care-team reminders and produce easy-to-read tip sheets for their clients. These tip sheets combine a personalized health status report and a practical, actionable and evidence-based health management plan-of-action. Follow-up appointments can be scheduled as needed to monitor progress and provide further guidance. Finally, a report is mailed to the patient's primary provider identifying risk factors and plans for modifying health risks.
